Recovering from a serious illness or injury can be a challenge, both physically and emotionally. Transitional Services at the M.M. Ewing Continuing Care Center include medically complex care and rehabilitation for those recuperating from an illness or surgery.
Our goal is to help individuals of all ages recover in the safety and security of our facility, and give them the confidence to resume their daily activities and return to the community.
Our team of caring, medical professionals work together to assess each client's unique needs to develop a course of therapy. The Interdisciplinary Care Planning (ICP) Team develops an individual care plan and monitors the client's progress and journey to optimal health.
The M.M. Ewing Continuing Care Center provides a friendly environment, offering clients who want to regain their strength and independence a sense of security and comfort while they recover and rehabilitate. The Skilled Transitional Unit offers both private and semi-private rooms. A carefully planned rehabilitation gym with the latest equipment and an occupational therapy apartment reinforce inclusion of the whole person in the recovery process.
Returning Home With the ICP Team, each client decides on the services needed after discharge. The planning may include a home visit with the team to see if special equipment or adjustments to the home living area are needed.
