At Thompson Health we are dedicated to providing exceptional quality, interdisciplinary care and education for medical nutrition therapy (MNT), to promote and support the health and well-being of the community.
Thompson's MNT program provides in-person and telemedicine outpatient visits to those with diabetes and chronic renal failure.
Individuals with diabetes and chronic renal failure may be referred by their Primary Care Provider (PCP) for a medical nutrition therapy consultation.
- Medicare and most insurances will cover 3 hours when initially (first year) diagnosed with diabetes mellitus (DM).
- One year after diagnosis, Medicare and most insurances will cover 2 hours per year for follow-up visits.
- Medicare and most insurances will cover medical nutrition therapy for patients with chronic renal failure that have a GRR of 13-50, when not on dialysis.
Scheduling an Appointment
To schedule an individual appointment, please have your PCP fax a referral to (585) 396-6915. Referrals should include the reason for MNT counseling. The most recent office visit notes, pertinent blood work, and any special needs of patients – e.g. Spanish-speaking, needs, wheelchair assistance, etc...
An MNT scheduler or registered dietitian will then call to schedule your consultation.
