CANANDAIGUA – Thompson Health recently learned that all five of its family practices have been selected as second-year participants in the Center for Medicare & Medicaid Innovation (CMMI) grant administered by the Finger Lakes Health Systems Agency.
Funded by the Patient Protection and Affordable Care Act, CMMI grants support promising new payment and care delivery models for Medicare, Medicaid and the Children's Health Insurance Program.
Through its selection as a participant, Thompson will significantly enhance its five family practices’ ongoing transformation to Patient-Centered Medical Homes (PCMH) certified by the National Committee for Quality Assurance. The PCMH care delivery model offers a comprehensive, team-based approach to care, focused on preventative care and resulting in greater patient satisfaction and lower hospitalization rates.
The funding received through the CMMI grant will provide trained RN care managers to assist all patients within each practice, a dedicated practice improvement consultant, an additional coordinator to work with the practices on social services integration and a financial award of $440,000 to go towards the practices’ transformation to PCMH.
