CANANDAIGUA – Finger Lakes Family Care (FLFC) recently joined UR Medicine’s Thompson Health, becoming the health system’s ninth primary care location in the region.
Located at 23 North St. in Canandaigua, FLFC was founded in 2006 by Dr. Robert L. Smith, who is joined by family nurse practitioners Leigh Lafferty and Erin Mahoney.
The practice is a “Patient-Centered Medical Home,” offering same day appointments for sick and acute visits, chronic disease management supervised by an in-house care manager. FLFC also offers secure email communications via Medent, as well as a Patient Portal, allowing patients secure, online access to health records, prescription refills and appointment requests.
FLFC was chosen by Excellus and MVP as one of seven practices in western New York for the Rochester Medical Home Initiative, and was also the first medical office in the Finger Lakes region to achieve national recognition as a Level 3 National Committee for Quality Assurance Patient Centered Medical Home.
